MiniPACS is a self-hosted PACS for independent radiology, imaging, veterinary and dental centers. It receives studies from any modality that speaks DICOM, stores them on one server in your building, and serves them to a browser viewer that cold-starts in a measured 0.6 seconds. Referring physicians and patients get expiring share links instead of discs and faxes, though it burns discs too when you need them. The price is $300 per location per month, flat: no per-study fees, no per-seat fees, no storage meters. The same engine already serves millions of images in production.

What MiniPACS does

  • DICOM archive on your hardware. C-STORE ingest from CT, MR, ultrasound, X-ray and the rest, indexed in PostgreSQL, with DICOMweb access and on-the-fly transcoding. The core rides on Orthanc, hardened and tuned for real concurrent clinic load.
  • Browser viewer and workspace. Studies open in any modern browser in 0.6 seconds, nothing installed. Compare mode with independent scroll, per-series download in DICOM or JPEG, and launchers into desktop viewers like OsiriX, RadiAnt and 3D Slicer when a workstation prefers them.
  • Live worklist. New studies appear on the worklist over a WebSocket push, no manual refresh. Filters live in the URL, so a colleague opens exactly the list you shared.
  • Report editor. Voice dictation with hands-free section jumps, dot-phrase macros, templates, autosave, and a full lifecycle from draft to signed release with addenda. Signed reports travel inside the study as DICOM encapsulated PDF, so they are never lost and never faxed twice.
  • Sharing and patient portal. Share links with expiry presets, PIN and QR, plus a branded guest portal where a patient or referrer views studies with nothing installed. Disc and USB export produces a self-contained ISO that opens even on an iPad.
  • Audit and HIPAA tooling. Every audit row is hash-chained and append-only at the database level, so nobody, including us, can quietly edit history. A self-service Accounting of Disclosures report answers patient access requests in one CSV export.
  • Backups and self-healing ops. Encrypted AES-256 backups on an in-UI schedule, downloadable by piece, plus a dual-channel self-updater with verified rollback and watchdogs for both Linux and Windows hosts. The backend has zero access to Docker or the host, so a compromise cannot reach your machine.
  • Admin control. An editable RBAC permission matrix, instant session revocation on sensitive changes, and white-label branding with your clinic logo.

What MiniPACS honestly does not do yet

Two gaps we state rather than hide: there is no Modality Worklist (MWL) server yet, so modalities cannot pull scheduled patient demographics from MiniPACS, and there is no built-in DICOM anonymization for research export. If either is a hard requirement today, we will tell you the same thing in a demo call.

Vendo, the referral and booking portal, is $500 per location per month standalone, and $640 per location per month for both products together.
