Safety gates most portals do not have

Vendo checks orders the way a careful technologist would, at submission time, on the server:

An order for contrast MRI with an eGFR under 30 is refused outright, on both the referrer path and the clinic path, instead of the problem surfacing on scan day.
Pregnancy requires a documented reason, not a click
An X-ray for a pregnant patient goes through only when the ordering physician records their clinical reasoning, and that reason stays on the chart permanently.
Allergy answers are captured fresh on every referral
Contrast-reaction history is asked per visit and stored per referral, so an answer from six months ago never silently stands in for today's.

Scheduling that cannot double-book

Availability is computed, not stored
Free time is derived live from working hours minus what is actually taken, so a stale free slot cannot exist. When an admin shortens clinic hours, the evening slots simply stop existing.
Slot claims are atomic
Two physicians clicking the same Thursday 2 p.m. slot at the same instant cannot both book it; the loser immediately sees that the time is gone, and a failed booking rolls the claim back automatically.
Daylight saving does not move appointments
The timezone engine keeps 9 a.m. at 9 a.m. across DST transitions, where naive offset math drifts an hour.

The audit trail uses DICOM and HL7 standard AuditEvent resources written under the acting user's own session token, so log entries cannot be forged without that user's access level. Server-side tenant guards re-check patient access on every request, closing the hole where a referrer could name another clinic's patient and gain read access. Uploads are validated by magic bytes, so a renamed executable pretending to be a PDF is caught before it touches disk. Sessions idle out honestly and renew silently while a user is actually working.
